The following forms are available as replacements if you wish to plead guilty to a traffic offense by mail, or if you wish to avoid multiple court appearances and plead not guilty/request a trial before your initial court date.

Of the plead of guilty forms, one form is a request for a conviction and the other is a request for court supervision. Please mail just one form along with your payment (no personal checks).

You do NOT need either form if you are pleading guilty and making your payment online, or if your case was previously handled in court.
