Several forms come with instructions for its use. While some forms were created specifically for use in the Twelfth Circuit Courts, state-approved standardized forms should be used where possible.

The Illinois Supreme Court has set out a formal process for the development, review and approval of standardized forms in all areas of law. These forms are for use by both self-represented litigants and attorneys. The forms are user-friendly and the format is approved for use by all courts in the state.
